Skin-sparing mastectomy (SSM) is oncologically safe and offers better cosmetic results than standard modified radical mastectomy (MRM). Recently, German investigators showed that SSM with nipple-sparing mastectomy (NSM; preservation of the nipple-areola complex) and immediate autologous reconstruction also was as safe as and aesthetically superior to MRM at mean follow-up of 59 months (Ann Surg 2003; 238:120). Now, these researchers present results at mean follow-up of 101 months. Recurrence and cosmetic results in 246 women who received NSM, SSM, or MRM were evaluated. Only 23% of patients had stage 0 or 1 disease, and >50% had axillary nodal metastases.
