Oxaliplatin (Eloxatin), a platinum-based chemotherapy agent used in colorectal cancer, is associated with a variety of neurological effects. Among these is acute neuropathy, which is characterized by cold-induced paresthesias and muscle spasms. These symptoms, which occur shortly after infusion, typically resolve in a few days, although chronic neuropathy can also result. The underlying cause of acute oxaliplatin-induced neuropathy is unclear; however, the rapid onset of symptoms suggests pharmacologic rather than structural etiology, and in vitro studies implicate alterations in axonal membrane sodium (Na+) channels.
