A therapy paradigm for metastatic renal cell carcinoma (MRCC) that involves sequential use of new agents, such as sunitinib and sorafenib, has evolved in practice despite limited prospective evidence. Although both sunitinib and sorafenib are tyrosine kinase inhibitors (TKIs) with multiple targets, the rationale for administering them in sequence can be justified because they have distinct pharmacokinetics and binding specificities for target kinases. To prospectively assess the activity of TKIs when used sequentially, Italian investigators conducted a multicenter, single-arm, phase II trial examining responses to sorafenib in patients who had disease progression after first-line sunitinib therapy for MRCC.
