Cholangiocarcinoma is relatively uncommon in developed countries. Most patients present with inoperable or metastatic disease; thus, few patients are candidates for curative surgery. Lack of data about therapeutic options for cholangiocarcinoma has led clinicians to adapt approaches used with pancreatic cancer patients. Although in that setting, combining gemcitabine with other agents generally has failed to improve outcomes over those with gemcitabine alone, gemcitabine-based combinations have shown promising results in patients with advanced cholangiocarcinoma in nonrandomized phase II studies. However, because such studies are flawed, no standard treatment exists for patients with metastatic cholangiocarcinoma.
