Recent clinical trials have supported the use of a platelet-count threshold of 10,000/mm3 and a low platelet dose per transfusion during treatment for hematologic malignancies (JW Oncol Hematol Feb 17 2010). To test a no-prophylaxis transfusion strategy, investigators conducted a randomized, multicenter, noninferiority trial involving 600 adults in the U.K. and Australia with hematologic malignancies who were undergoing chemotherapy or stem-cell transplantation (SCT). Patients in the control arm received prophylactic platelet transfusion if the morning platelet count was <10,000/mm3, whereas those in the experimental arm were transfused only for a bleeding event.
