Anticoagulant therapy is routinely used in young women after cardiac valve replacement; however, heavy menstrual bleeding and even anemia can result. Turkish investigators evaluated the effectiveness of the levonorgestrel-releasing intrauterine device (LNG-IUD) versus observation in a randomized trial involving 40 women with menorrhagia (determined by scores on a standardized pictorial bleeding assessment chart [PBAC]) who had undergone valve replacement.
At 3 and 6 months after LNG-IUD insertion, women had significantly lower PBAC scores and higher hemoglobin concentrations (e.g., 10 g/dL before insertion vs. 12 g/dL at 6 months), hematocrits, and ferritin levels than they did before insertion; levels in the observation group remained uncha…
