Most cases of gestational hypertension occur after 36 weeks' gestation, but the best timing of delivery in this setting remains unknown. In a retrospective cohort study, investigators used data from the Consortium on Safe Labor to determine the optimal timing of labor induction in 3588 women with gestational hypertension (defined as blood pressure ≥140/90 without proteinuria after 20 weeks' gestation) and singleton first pregnancies of ≥36 weeks' gestation. Maternal and neonatal outcomes were assessed at gestational ages at delivery ranging from 36 to 42 weeks and included a composite of outcomes (e.g., placental abruption, postpartum hypertension, small for gestational age [SGA], cesarean delivery) or death.
