Confrontation visual field testing is a rapid and important component of a neurological examination. In this study, investigators examined the reliability of seven commonly used confrontation-testing techniques compared with the “gold standard” of 24-degree automated (Humphrey) perimetry. They prospectively studied 163 patients from a neuro-ophthalmology clinic, after excluding those with poor reliability measures on perimetry.
Individual confrontation techniques all showed limited sensitivity and specificity. The kinetic red target test performed best in isolation (achieving 74.4% sensitivity and 93.0% specificity), and its reliability improved when combined with other techniques (such as finger wiggling or face description).
