The risk of acquiring HIV from performing oral sex on men (fellatio) has been difficult to assess, with quantitative estimates drawn largely from small case series. One study reported a per-contact risk for infection of 0.04% from unprotected fellatio with a partner whose serostatus was either unknown or positive. A study among high-risk individuals in San Francisco, however, reported that 8.0% of infected participants acquired HIV from fellatio. Given this wide variation, it is difficult to know how to counsel patients on the risk associated with performing fellatio.
