It has been generally accepted that bilious vomiting in a newborn implies intestinal obstruction, but there are scant data to support this contention. To examine this hypothesis, researchers prospectively evaluated children with a history of bilious vomiting who were referred to a pediatric surgical unit during a 2-year period and followed them after discharge.
In all, 63 neonates (35 males) in whom "green vomiting" was witnessed by an experienced observer were admitted. The median gestational age was 40 weeks (range, 31-42), and the mean weight was 3.5 kg (range, 1.67-4.64). Median age at presentation was 26 hours (range, 9 hours to 28 days). Fifty infants had been delivered vaginally, and 49 had passed meconium within 24 hours after birth.…
