Infarction in the anterior choroidal artery (AChA) territory has rarely been studied. These authors analyzed the clinical, etiologic, and prognostic profile of 42 patients with AChA-territory infarcts, from a total of 1057 patients in a prospective registry of acute stroke (818 ischemic strokes, 120 transient ischemic attacks, and 119 intracranial hemorrhages).
Of the 42 patients with AChA infarction, 88% had a lacunar syndrome. Only 14% of strokes were attributed to presumed small-artery disease, 38.1% to large-artery atherothrombosis, 38.1% to a cryptogenic cause, and 4.8% to presumed cardioembolism.
