Nocturnal acid breakthrough (NAB) has been defined arbitrarily as intragastric pH <4 for longer than 1 hour at night during treatment with proton-pump inhibitors (PPIs). Although an additional nocturnal dose of a histamine-2-receptor antagonist (H2RA) has been shown to provide additional acid control, the clinical significance of this approach has not been studied. Researchers prospectively evaluated intragastric and esophageal pH associated with antireflux regimens in 13 patients with gastroesophageal reflux disease (GERD) and in 9 healthy controls.
