The hazards of diltiazem, a calcium channel blocking agent, in patients with left ventricular dysfunction after myocardial infarction were confirmed by a long-term follow- up study from the Multicenter Diltiazem Postinfarction Trial (MDPIT). MDPIT was a randomized, double-blind, placebo- controlled trial of diltiazem in 2466 postinfarction patients followed for 12 to 52 months. Previous MDPIT reports have indicated that diltiazem may lead to decreased mortality and nonfatal reinfarctions in patients with good left ventricular function, but may increase these events in patients with left ventricular impairment.
