Although platelets and fresh frozen plasma (FFP) transfusions frequently are required in critically ill patients, institutions and individual clinicians use various transfusion indications and thresholds. Furthermore, platelets and FFP are scarce and costly resources that can cause substantial complications. In this guideline, the authors attempt to consolidate the evidence for FFP and platelet transfusions in critically ill patients.
Initial considerations:
In stable, nonbleeding patients with thrombocytopenia but without high risk for spontaneous bleeding, platelet transfusion is suggested when levels drop below 10,000/µL.
