Endoscopy is the standard of care for the evaluation and assessment of gastroesophageal reflux disease–related erosive esophagitis. Although several systems exist for grading erosive esophagitis, none allows definitive interpretation by endoscopists. This limitation is particularly evident in milder grades of esophagitis, because of uncertainty in accurately detecting small mucosal breaks and assessing their lengths. For example, in the Los Angeles grading system, considerable variation is seen between observers in defining grade A versus grade B esophagitis.
