Gestational diabetes mellitus (GDM) can lead to larger fetuses, but such increases in fetal weight can be prevented with insulin therapy. To determine whether effective treatment for GDM also leads to lower rates of delivery by cesarean section, these Canadian researchers studied 3778 pregnant women, of whom 143 had known, treated GDM and 115 had untreated, borderline GDM.
Women with overt or borderline GDM had higher rates of previous cesarean delivery (17 and 14 percent, respectively) than women without GDM (8.5 percent). Only 10.5 percent of those with treated GDM had infants with a birth weight greater than 4000 grams, compared with 29 percent of those with untreated GDM and 14 percent of those without GDM.
