Steroid Injections in PI Recipients Can Precipitate HPA Axis Dysfunction

In a retrospective analysis, hypothalamic-pituitary-adrenal axis dysfunction developed after steroid injections in 11% of HIV-infected patients taking protease inhibitors, but in no patients not taking these agents.

Protease inhibitors (PIs), which inhibit the cytochrome P450 3A4 (CYP3A4) pathway, interact with many medications that are commonly used to treat comorbid medical conditions. As patients with HIV infection live longer, polypharmacy and the resulting complications are important considerations for HIV providers.

In a retrospective analysis of data from a healthcare system in Boston, investigators examined the frequency of hypothalamic-pituitary-adrenal (HPA) axis dysfunction in HIV-infected adults who received intra-articular steroid injections between 2002 and 2011.
