Patients with Crohn disease have a high rate of postoperative recurrence, which few medical therapies can effectively prevent. Infliximab is one of them: A small, randomized, controlled trial of infliximab showed a dramatic effect in reducing recurrent Crohn disease compared with placebo (NEJM JW Gastroenterol Apr 3 2009). Now, researchers have tested the efficacy of adalimumab as a prophylactic therapy in this setting.
Investigators randomized 51 patients undergoing surgery for ileal or ileocolonic Crohn disease to one of three treatment arms: adalimumab (40 mg every 2 weeks), azathioprine (2 mg/kg per day), or mesalamine (3 g per day).
