Histoplasmosis is a common fungal infection in the Ohio and Mississippi River valleys. Severe disease is more common in individuals with suppressed T-cell immune function, as occurs after solid-organ transplantation (SOT). Active histoplasmosis in patients who have undergone SOT may occur from a new primary infection, from a second exposure to the fungus during immune suppression, or from reactivation of infection acquired prior to transplantation. Rarely, the transplanted organ is the source of infection.
