Perioperative myocardial ischemia often goes undetected. In this multicenter, prospective cohort study, researchers sought to establish criteria for myocardial injury after noncardiac surgery (MINS) that would predict 30-day mortality. About 15,000 patients (age, ≥45) who underwent inpatient noncardiac surgery had fourth-generation troponin T testing at 6 to 12 hours after surgery and daily for 3 days after surgery. Patients with troponin T levels ≥0.04 ng/mL underwent electrocardiography (ECG) and were asked about ischemic symptoms. Patients whose elevated troponin levels were considered to be secondary to nonischemic events (e.g., sepsis, pulmonary embolism) were excluded.
