Sarcoidosis is a multisystem disorder that involves the skin in about 20% of patients. The skin may be the primary organ affected. Face involvement is disfiguring and impacts quality of life. Few controlled studies of agents for cutaneous sarcoidosis are available. Thalidomide has been proposed as a potential therapy based on observations in single cases or small series. These authors performed a double-blind, randomized trial of 100 mg daily of thalidomide or placebo in 39 patients with severe disfiguring cutaneous sarcoidosis. Recipients of stable doses of prednisone of ≤15 mg/day who had not benefited from prior cycline antibiotics, hydroxychloroquine, or immunosuppressive agents were included.
