The search for a nonsurgical alternative to treat severe mitral regurgitation (MR), particularly for patients with surgical contraindications, has been energized by the successes of transcatheter aortic valve replacement (TAVR) and MitraClip transcatheter repair of MR. Investigators now report the first results with a novel transcatheter mitral valve implantation (TMVI) system (Tiara). The device is a nitinol, self-expanding, trileaflet, bovine pericardial prosthesis that is inserted transapically with a 32-French delivery device and anchors to the trigones, leaflets, and posterior annulus. Two of the authors report financial ties to the device manufacturer.
