Many patients with severe aortic stenosis have ventricular hypertrophy and diastolic dysfunction. The use of angiotensin-converting enzyme inhibitors (ACEI) or angiotensin-receptor blockers (ARBs) is associated with improvements in left ventricular remodeling and hypertrophy in patients with heart failure. To ascertain the effect of renin–angiotensin system (RAS) blockade on outcomes after surgical aortic valve replacement (SAVR) for severe aortic stenosis, investigators from the Cleveland Clinic conducted a retrospective, observational study of 741 patients who received RAS blockade after SAVR and 1011 SAVR patients who did not.
