Data are sparse regarding optimal timing of delivery in growth-restricted pregnancies with elevated umbilical artery systolic/diastolic (S/D) ratios (an ultrasound-based indicator of increased placental resistance). Current guidelines, based largely on expert opinion, recommend delivery at 37 weeks if S/D ratios are elevated and 38 to 39 weeks if ratios are normal. In a single-institution retrospective study of 512 growth-restricted fetuses, researchers assessed neonatal outcomes in those with elevated S/D ratios (delivered at 37 weeks' gestation) compared with those having normal S/D ratios (delivered at 39 weeks).
