The Veterans Affairs Diabetes Trial (VADT; NEJM JW Gen Med Jun 15 2009 and N Engl J Med 2009; 360:129) was one of three randomized trials (the others were ACCORD and ADVANCE; NEJM JW Gen Med Jul 1 2008 and N Engl J Med 2008 Jun 6; [e-pub]) in which intensive glycemic control did not lower rates of major vascular events in older patients with long-standing type 2 diabetes. Median follow-up during the active portion of VADT was 5.6 years. Now, researchers report overall cardiovascular outcomes about 5 years after the trial ended; during this post-trial period, difference in glycemic control between the intensive and standard-care groups gradually narrowed.
