The chances of a successful trial of labor after cesarean delivery (TOLAC) are better if the woman has had one prior vaginal delivery and the fetus weighs <4000 gm. However, the effect of induction of labor on TOLAC is not fully understood. In a retrospective study of data from the Consortium on Safe Labor, TOLAC success rates for singleton pregnancies at 37 to 40 weeks' gestation were assessed in 1626 women undergoing induction of labor compared with 4407 women managed expectantly as they awaited spontaneous labor. All women had one prior cesarean delivery and all study births occurred between 2002 and 2008, predating professional society guidance to avoid elective induction before 39 weeks.
