The adenoma detection rate (ADR) — the proportion of screening colonoscopies in patients aged ≥50 years in which one or more conventional adenomas is detected — is considered the single most important quality measure in colonoscopy. Two large studies have demonstrated that higher ADRs are associated with lower postcolonoscopy incidence of colorectal cancer.
However, some have raised concern that higher ADRs are achieved largely through detection of greater numbers of small adenomas, which leads to shorter surveillance intervals and more colonoscopies per unit time.
