Pyoderma gangrenosum, an ulcerative disease of unknown etiology, is associated with underlying disease, primarily inflammatory bowel disease and inflammatory arthritis, about 35% of the time. Many therapies have been proposed, but few have been studied in randomized, controlled trials.
These authors conducted a multicenter, randomized, controlled, blinded comparison of prednisolone and cyclosporine in 112 patients with a diagnosis of pyoderma gangrenosum. Many patients had not been biopsied due to concerns about pathergy. Patients received prednisolone 1 mg/kg/day, up to 75 mg daily, or cyclosporine 4 mg/kg/day, up to 400 mg daily.
