Atopic dermatitis (AD) can be severe and require systemic therapy for its management. Systemic corticosteroids are effective but are frequently associated with adverse reactions and are noted for tachyphylaxis (diminished response) with continued usage. Various immunosuppressive agents have been used successfully, including azathioprine, mycophenolate, cyclosporine, and methotrexate, in some patients with AD. The pathogenesis of AD is becoming clearer and is generally believed to involve Th2 immune cells. Some evidence also suggests that Janus kinase (JAK) inhibitors might play a role in down-regulating the relevant pathway.
