Traditionally, face-to-face visits with patients have been required for practices to bill for care. In January 2015, Medicare initiated chronic care management (CCM) payments of US$40 per month for non–visit-based care of Medicare fee-for-service patients with ≥2 chronic conditions. This non–visit-based care can be delivered by nonphysicians and must be ≥20 minutes' duration. In a modeling study, researchers estimated the financial implications of CCM payments for primary care practices.
