A regimen of sofosbuvir (NS5B inhibitor) plus simeprevir (protease inhibitor) for 12 to 24 weeks is currently FDA-approved for the treatment of hepatitis C virus (HCV) genotype 1 infection but is not well studied in patients with advanced cirrhosis.
In a retrospective study of prospectively collected data, investigators assessed the efficacy of sofosbuvir (400 mg) plus simeprevir (150 mg) daily with or without ribavirin (at physician discretion) for 12 weeks in treating 119 patients with HCV genotype 1 and compensated cirrhosis or decompensated cirrhosis (undergoing liver transplant evaluation). The primary endpoint was sustained virologic response 12 weeks after treatment cessation (SVR12).
