Management of uncomplicated acute diverticulitis is currently being reconsidered. In a recent technical review of this topic for the American Gastroenterological Association (AGA), researchers classified essentially all the evidence as very low quality based on Grading of Recommendations Assessment, Development and Evaluation (GRADE) criteria. Their conclusions include the following:
The lifetime risk for diverticulitis in patients with diverticulosis is about 5%.
About 20% of patients with acute uncomplicated diverticulitis will have a recurrence, and most recurrences are not complicated.
Routine use of broad-spectrum antibiotics for uncomplicated acute diverticulitis may not reduce symptom duration but may reduce complications and recurrence…
