Management of pregnant women with histories of bipolar disorder or postpartum psychosis is challenging. To study relapse risks and associated factors, investigators conducted meta-analyses on postpartum relapse risks in women with bipolar disorder (25 studies involving 5105 deliveries in 3495 patients) and women with postpartum psychosis histories (13 studies involving 595 deliveries in 528 patients).
In bipolar disorder overall, relapse rate was 37%, and rates did not differ significantly between bipolar types I and II. For bipolar women with prior postpartum episodes, relapse risks ranged from 87% in the largest series (45/52 patients) to 41% in smaller studies.
