Atypical antipsychotics, benzodiazepines, and antidepressants are often used to address aggression and agitation in patients with Alzheimer disease (AD), despite limited evidence of efficacy and considerable potential adverse effects. To better assess the value of the selective serotonin reuptake inhibitor (SSRI) citalopram, investigators analyzed data from CitAD, a 9-week, NIMH-funded randomized trial, involving 186 AD patients, in which moderate or better improvement in agitation was significantly more common with citalopram (≤30 mg/day) than with placebo (40% vs. 26% of participants; NEJM JW Psychiatry Apr 2014 and JAMA 2014; 311:682).
