The data on the benefits of position change during colonoscopy for improving adenoma detection are mixed. The idea of changing positions is to move dependent portions of the colon into nondependent positions so that they hold gas and distend better.
In the current randomized trial involving 1072 patients undergoing colonoscopy, the cecum, ascending colon, and hepatic flexure were examined in the left lateral decubitus position, the transverse colon in the supine position, and the left colon in the right lateral decubitus position. In the control group, patients remained in the left lateral decubitus position throughout the procedure. Patients were sedated with midazolam with or without meperidine. Results were as follows:
