The sodium–glucose cotransporter-2 (SGLT2) inhibitor canagliflozin (Invokana) is FDA-approved for use only in patients with type 2 diabetes. However, anecdotal reports suggest that it is prescribed off-label for type 1 diabetic patients and that it can increase risk for diabetic ketoacidosis (DKA) in such patients (NEJM JW Gen Med Oct 15 2015 and Diabetes Care 2015; 38:1687). Now, researchers describe the incidence and characteristics of DKA among 351 type 1 diabetic patients (mean age, 42) who were randomized to receive 18-week courses of canagliflozin or placebo, added to insulin; general results of this industry-sponsored trial were reported previously (Diabetes Care 2015; 38:2258).
