Sulfonylureas are associated with risk for hypoglycemia, but how this risk is affected by kidney function and type of sulfonylurea used is unknown. In this population-based cohort study, researchers used data from U.K. general practices to compare risk for hypoglycemia (defined as a coded diagnosis of hypoglycemia or a documented blood glucose level <54 mg/dL) in sulfonylurea users with that in metformin users; 121,000 new users (mean age, 67) of noninsulin antidiabetes medications were categorized according to kidney function and sulfonylurea dose and type. Median follow-up was 3.7 years.
