For patients with acute ischemic stroke who receive tissue plasminogen activator (TPA), antithrombotic agents such as aspirin customarily are withheld until 24 hours after TPA administration because of concern about triggering hemorrhagic transformation. More recently, acute stroke intervention with endovascular thrombectomy is being performed (with or without TPA), and data on early antithrombotic use in these patients are limited.
These authors evaluated an acute stroke database of 712 patients treated during an 8-year period. Among patients who received early antithrombotic treatment (<24 hours after thrombolytic therapy), 35% received intravenous TPA only, 38% received endovascular therapy only, and 28% received combined therapy.
