Guidelines recommend use of endovascular therapy only for proximal occlusions of the middle cerebral artery or the distal internal carotid artery. This retrospective multicenter study examined whether endovascular therapy benefits more-distal occlusions. The authors compared some 522 patients with occlusion in the M2 segment of the middle cerebral artery (defined as the vertical segment lying medial to the sylvian fissure) who received either endovascular treatment (EVT) or medical therapy. Patients had a mean NIH Stroke Scale score of 16, and 74% of the medically treated patients received intravenous tissue plasminogen activator, versus 60% of the EVT patients.
