External cephalic version (ECV) of breech fetuses can lower cesarean delivery rates substantially. The likelihood of successful ECV is enhanced by pretreatment with a β-agonist to provoke uterine relaxation (Cochrane Database Syst Rev 2015; 2:CD000184); however, because these agents have cardiovascular side effects (e.g., palpitations), alternative pharmacologic agents such as oxytocin antagonists are important to study. Dutch investigators randomized 830 women with singleton breech presentations at >34 weeks' gestation to a single intravenous dose of the oxytocin antagonist atosiban (6.75 mg) or the β-agonist fenoterol (40 µg) given 15 minutes before ECV was attempted.
