In a prospective cohort study, investigators evaluated the impact on emergency department (ED) visits and hospitalization of an enriched medical home intervention at five pediatric primary care offices affiliated with a university-based children's medical center in New York. The Keeping Families Healthy model integrates community health workers into ED visits and hospitalizations with the goal of reducing potentially avoidable hospital utilization for ambulatory care–sensitive conditions.
