The CDC has issued a health advisory describing an increase in the number of reported cases of infections due to Shigella strains that have reduced ciprofloxacin susceptibility, with a minimum inhibitory concentration (MIC) level for ciprofloxacin of 0.12–1 μg/mL (typical Shigella ciprofloxacin MIC, ≤0.015 μg/mL). Many of these new strains are reported to contain plasmids with at least one gene that confers ciprofloxacin resistance. Although no information is available on the clinical outcome of infections due to reduced ciprofloxacin-susceptible Shigella isolates treated with ciprofloxacin, the clinical outcome has been worse with Salmonella infections due to Salmonella species that have ciprofloxacin MICs of 0.12–1 μg/mL.
