Evidence and guidelines support performing carotid endarterectomy (CEA) within 2 weeks following an ischemic stroke caused by carotid artery stenosis if the patient does not have a severe or unstable neurologic condition. Evidence on the risks and benefits of early carotid artery stenting (CAS) has not been as clear. To compare the outcomes of early revascularization with CEA or CAS, investigators compiled the results from four multicenter trials (3 in Europe, 1 in North America).
All 4138 patients were symptomatic, with a recent stroke or transient ischemic attack. Very few patients had the revascularization procedure done within 7 days of the last neurologic event (14% with CAS, 11% with CEA). The median delay to revascularization was 26 d…
