Coagulation Factor Concentrates Reduce Hemorrhage in Traumatic Coagulopathy

Compared with fresh frozen plasma, fibrinogen and clotting factor concentrates reduce bleeding and transfusion needs in trauma patients.

In an open-label randomized trial, researchers assessed whether use of fibrinogen concentrates in addition to coagulation factor concentrates (CFC) reduces hemorrhage in patients with traumatic coagulopathy, when compared with fresh frozen plasma (FFP). At a single trauma center in Austria, patients with significant trauma (Injury Severity Score >15) and evidence of major hemorrhage were screened for coagulopathy using thromboelastometry. Those with coagulopathy were randomized to receive either FFP or CFC (fibrinogen concentrate plus factor XIII concentrate, prothrombin complex concentrate, or both, depending on the abnormality identified on thromboelastometry).
