Although carotid revascularization is performed frequently, less attention has been devoted to surgical or endovascular treatment of vertebral artery (VA) stenosis. Previous studies of VA stenting have shown a relatively high complication rate, a high restenosis rate, or both.
The VIST multicenter trial was conducted at 14 hospitals in the U.K. Patients with a recent posterior circulation stroke or transient ischemic attack (TIA) and ≥50% VA stenosis, either at the vessel origin or intracranially, were randomly assigned to receive best medical treatment (BMT) alone or BMT plus angioplasty/stenting. The primary outcome was stroke in any arterial territory.
