Monoclonal antibodies targeting the PD-1 receptor and PDL-1 ligand typically have little or no activity in metastatic colorectal cancer (CRC). However, early patient series have indicated that microsatellite instability (MSI) is a biomarker of response to these agents.
Investigators now report results of a large, industry-sponsored, international, open-label phase II trial (CheckMate 142) of the PD-1 immune checkpoint inhibitor nivolumab in 74 patients (median age, 53 years) with either MSI-high or DNA mismatch repair-deficient (dMMR) metastatic CRC. All patients had progressed on at least one prior line of chemotherapy, and most (54%) had progressed on three or more prior lines. Patients were given nivolumab (3 mg/kg every 2 weeks).
