Previous case reports describe appendicitis and diverticulitis occurring shortly after colonoscopy. In a study of approximately 400,000 U.S. veterans undergoing colonoscopy, researchers compared the incidence of appendicitis in week 1 after colonoscopy versus weeks 2 to 52. After excluding patients with preexisting symptoms, appendiceal neoplasms, incidental appendectomies, or no appendectomy in the operating note after chart review, there was a higher incidence of appendicitis in the first week after colonoscopy (7 cases) compared with weeks 2 to 52 postprocedure (79 cases; incidence rate ratio, 4.5).
