Vancomycin is one of the most commonly prescribed antibiotics in hospitals, used in both empiric and directed treatments against beta-lactam–resistant Gram-positive organisms. Increased use in recent years has rekindled concerns of nephrotoxicity that were more common in less-purified formulations decades ago, particularly with concomitant aminoglycosides, piperacillin-tazobactam, or other nephrotoxic medications.
