Increasing rates of sexually transmitted infections (STIs) among men who have sex with men (MSM) have been noted, in particular among those on tenofovir/FTC pre-exposure prophylaxis (PrEP; NEJM JW Infect Dis Jan 2016 and N Engl J Med 2015 Dec 3; 373:2237). To assess if doxycycline given after unprotected sex (post-exposure prophylaxis [PEP]) decreased the incidence of STIs, investigators in France performed an open-label trial in which they randomized 232 participants taking tenofovir/FTC on-demand for PrEP 1:1 to either no prophylaxis or doxycycline (two 100-mg pills taken within 24 hours and no later than 72 hours after sex). The primary endpoint was the occurrence of a first STI (syphilis, chlamydia, or gonorrhea) after randomization.
