Congestive heart failure (CHF) and atrial fibrillation (AF) often coexist, but only recently have we gotten guidance about therapeutic approaches, with rhythm-controlling strategies with amiodarone and dofetilide found not more effective than rate control. Since 2008, randomized controlled studies have shown clinical benefit of AF ablation compared with other strategies. Now, in an industry-supported study, researchers have compared AF ablation with guideline-driven medical therapy for rate or rhythm control in 363 patients with paroxysmal or persistent AF (New York Heart Association class II–IV for CHF; left ventricular ejection fraction [LVEF], ≤35%), who were unresponsive, intolerant, or unwilling to take antiarrhythmic medications.
